This payment form is for those who have been accepted into the ICMA Summer Intensive.

The undersigned is the adult-aged student, parent, or legal guardian of a child planning to participate in an educational program of ICMA. I grant to ICMA, its representatives, and employees the right to take photographs and video recordings of me (if over 18)/my child and my property in connection with the above-identified subject. I authorize ICMA, its assigns, and transferees to copyright, use, and publish the same in print and/or electronically. I agree that ICMA may use such photographs and/or video recordings of my child with or without his/her name and for any lawful purpose, including, for example, publicity, illustration, advertising, and web content.
